If your license is from New Hampshire, Pennsylvania, or Washington, you will need to fill out and submit your state's records form. Please bring complete forms to Parking and Transportation Services at 1213 Kipke Drive.
You will not be able to drive unless you follow these instructions.
New Hampshire:
A signed, notarized affidavit (download) will need to be completed and returned with your driver application. This is the law in the State of New Hampshire and PTS will not be able to obtain a copy of your record without this form. Please see the attached instructions.
Pennsylvania:
A signed, notarized affidavit (download) will need to be completed and returned in addition to your driver application. You need to fill out the following sections:
- A: Requester Information
- C: Driver Information
- D: Affidavit of Intended Use
- E: Driver Release
Washington:
A signed affidavit (download) will need to be completed and returned in addition to your driver application. Fill out everything up to Employer Attestation, including name of driver, signature, date and WA license number.
